Treatment of periodontal diseases

Traditional dental cleanings are a vital tool towards preventing and treating gum disease. As periodontal disease progresses, however, cleanings are limited due to the inability to access and remove the bacteria residing deep underneath the gums. Cleaning underneath the gums (deep cleaning) is a procedure that can be very sensitive to a patient and is thus generally performed under local anesthesia. Following deep cleaning, a healing period of 4-8 weeks is allowed before re-evaluating the condition of the gums. After this re-evaluation, a decision can be made as to whether the periodontal condition is stable or if further treatment (surgical) is indicated.

Periodontal surgery

Periodontal surgery is a procedure indicated in moderate to severe cases that cannot be treated adequately with deep cleaning alone. Pocket reduction surgery involves cutting away a small portion of the gum tissue and gaining access to the underlying bone, which may require smoothing in areas where bacteria caused bone destruction. A drawback to the procedure is that the teeth will appear slightly longer, but this is necessary when trying to reduce the pockets and restore the gums to a state of health.

Periodontal regeneration

Gum disease has traditionally been treated by trimming away the infected tissue and by re-contouring the uneven bone. Although this is an effective way of treating periodontal disease, new and more sophisticated regenerative procedures can be used to regenerate bone around teeth that has been lost due to periodontal disease. This is only possible in certain cases, stressing the importance of detecting and treating periodontal disease early on before the disease is too advanced to treat.

Dental implant

Dental implants are screws that are placed into the upper or lower jawbone. Once left to heal (healing time is anywhere from 3 weeks to 6 months) they are used to support a restoration (crown, bridge or denture). Dental implants are usually made out of titanium or a titanium alloy as the body tolerates this metal very well. The biggest prerequisite for implants is sufficient amount of bone to stabilize the screw. In cases where there isn’t enough bone present, bone grafting may be required before or at the same time as implant placement.

Treatment of peri-implant diseases

Peri-implantitis is a disease that affects dental implants. Its characteristics include bone loss, deep probing depths, bleeding, suppuration (pus), swelling and in few cases pain. It is a destructive inflammatory process that affects both the soft (gums) and hard (bone) tissues around the dental implant. Treatment for peri-implantitis includes non-surgical and surgical decontamination of the implant and bone regeneration.

Soft tissue grafting

Soft tissue grafts, often called gum grafts, are a very common procedure with a variety of uses. Two main types of soft tissue graft exist, and each has a different indication. The connective tissue graft (CTG) is most commonly used to cover the exposed root of a tooth that has undergone recession. This is a very predictable procedure, although there are limitations that must be evaluated and discussed with the patient before starting surgery. The free gingival graft (FGG) is type of gum graft that is used most commonly to increase the thickness and quality of tissue in areas where it is thin. Thin tissue is much more prone to recession and inflammation and thus a soft tissue graft is often prescribed in these areas to prevent future recession. Free gingival grafts are also indicated around crowns, dental implants or prior to orthodontic treatment if the gum tissue is too thin.

Bone grafting

Bone grafting is used to regenerate bone that has been lost as a result of tooth extraction. Following the extraction of a tooth, the surrounding bone loses its support (the tooth) and will collapse in on itself over time. The longer an extracted tooth site is left, the more bone loss occurs. To minimize this bone loss from extraction before it happens, bone grafting is often done at the same time as tooth removal, a procedure called “site preservation”. This ensures that once healing from an extraction is complete, there will be sufficient bone to stabilize a dental implant.

Sinus augmentation

The maxillary sinus is an air-filled cavity in the posterior part of the upper jaw. When planning implant placement in such sites, there is often an insufficient height of bone to support an implant and a sinus elevation procedure is needed. To perform this procedure, the thin membrane that lines the walls of the sinus is raised up slightly by packing bone graft material underneath it, thus slightly decreasing the size of the sinus and increasing the amount of bone below it.

Exposition of impacted tooth

Many adults end up with one or more teeth that fail to erupt normally and get stuck under the gum line. Often, orthodontic treatment will be required in order to erupt normally these buried teeth. A small surgery under local anesthesia is often necessary in order to expose the buried tooth and bond a small anchor to it which will be used to make the tooth erupt gradually by means of orthodontic treatments.

Accelerated orthodontics

With the Accelerated Osteogenic Orthodontics (AOO) procedure most orthodontic cases in both adolescents and adults can be completed 3 to 4 times faster than would be possible with conventional orthodontics. Often 6-9 months of total time in braces! This is accomplished by combining efficient orthodontic mechanics by an orthodontist and alveolar decortication and augmentation techniques
